Lasts one to four hours, I guess? Kind of hard to tell if it's hyperfocus or time blindness for durations under two hours.

At work, one or two times a week, though the distinction between hyperfocus and flow state is a bit squiffy there. At home, maybe three to five times a week, usually while playing videogames but sometimes while doing yardwork.

It depends on how interested I am or can successfully convince myself to be in any given thing (if that makes sense). If it happens when working on a computer, I always feel like shit once it stops for some reason; almost completely worn-out. The hyperfocus can last for anywhere from 45 minutes to 10 hours, depending on how long I can go without an abrupt distraction/intervention. It's a blessed curse or a cursed blessing.

Multiple times a day for me. Sometimes just a few minutes on small simple things, other times it's hours playing a game or watching TV.
