The conspicuity is the point. It's like any other body modification; it connotes social class/rank. The same way a prison tattoo let's people know what you're about, this type of plastic surgery let's people know what you're about. It's not intended to be convincing or subtle. It's to set them apart from the steerage class.

I wish we lived in a world with people that complicated. I wish there were evil masterminds on even the most basic level, because it would imply a greater potential for our species in all regards.
These old marionettes are suffering from a basket of mental health issues stemmed from decades... maybe centuries, of constant approval seeking from a world of only one kind of man, the only kind they will ever be around so they can only imagine a dimension of sneering and leering and being literally abandoned and alone for displaying anything but the most over-top display of cartoonish, 2-dimensional caricature of femininity, because any variation or subtly in displaying gender identity is not allowed in the superficial hellworld deep within the warp that is conservative celebrity.
